j'ai eu le même problème sur des rails 4.2 fixé par

Pour Les Rails > 4.1

1) Ajouter PG bouncer buildpack de heroku

heroku buildpacks:add https://github.com/heroku/heroku-buildpack-pgbouncer

2) Ajouter PGBOUNCER_PREPARED_STATEMENTSfalse dans la variable d'environnement heroku.
